2024-2025 New York State Health Requirements
All students entering 3rd and 5th grade are required by New York State to have an updated Physical Exam on file in the Health Office. Physicals must be on this form.
The physical can be dated from 9/1/23 forward to the current date.
Please fax to (585)242-5156, mail, email, or bring to the Health Office.
Please schedule an appointment with your child's medical provider as soon as possible.
Immunization requirements for attendance are as follows:
New York state only allows a 14-day grace period after the start of the school year to receive mandatory vaccines. For your child to continue participating in school after the 14 allotted days, your child needs to receive their required vaccine with documentation submitted to the FRES Health Office.
It's best to check with your medical provider to see whether your child needs any vaccines.
The Monroe County Health Department has an Immunization Clinic.
Please call (585) 753-5150 for more information.
The FRES Health Office can be reached by phone at
(585) 242-5140 ext. 2801 and by fax at (585) 242-5156.
The office is open while school is in session.
