Health/Nurses Office

The nurses at FRES are Ann Clark, RN and Megan Koopman, RN. Our office provides health services for students, and we act as advocates, liaisons, and consultants on health-related issues for students, families, and school personnel. We also help support an optimum physical, mental, emotional, and social climate for all students and staff.

2024-2025 New York State Health Requirements

All students entering 3rd and 5th grade are required by New York State to have an updated Physical Exam on file in the Health Office.  Physicals must be on this form.

The physical can be dated from 9/1/23 forward to the current date.

Please schedule an appointment with your child's medical provider as soon as possible.


Immunization requirements for attendance are as follows:

  • Three (3) doses Diphtheria (DPT, DT or Td)
  • Three (3) doses Polio
  • Two (2) doses MMR (Measles, Mumps, & Rubella); the first shot must be on or after the child's first birthday
  • Three (3) doses Hepatitis B
  • Anyone born on or after January 1, 1994, must have varicella (chicken pox) immunization or physician documented proof of varicella disease.
  • All entrants or re-entrants who have lived or traveled in Africa, South America, Asia, the Near, Middle or Far East must also provide proof of freedom from or immunity to tuberculosis (TB) within one month prior to attending school. This proof may be in the form of a documented negative PPD administered in the USA in the past 12 months or a documented chest x-ray, negative for TB obtained within the past 12 months.

New York state only allows a 14-day grace period after the start of the school year to receive mandatory vaccines.  For your child to continue participating in school after the 14 allotted days, your child needs to receive their required vaccine with documentation submitted to the FRES Health Office.


It's best to check with your medical provider to see whether your child needs any vaccines.

The Monroe County Health Department has an Immunization Clinic.
